Course Reserves Request Form

Use this form for placing books or videos from the Library's collection on course reserve. See the course reserves policy for details.

If you are submitting a personal copy of a book for reserves, do NOT use the form below. Instead use this link to print a blank copy of the form, fill it out by hand and submit it to the Circulation Desk along with your reader or personal copies.

If neither you nor the Library owns the book, complete a suggestions for purchase form and indicate in the notes that, if possible, you would like the book placed on reserve when it arrives. Please be aware that the Library may not be able to purchase the item in time for your course.

You can use the Library's online reserves system, ERes, for articles, book chapters, web pages and electronic documents.
